4. Prepare Your Child’s Immunization Record
   Take a clear, sharp image of your child’s immunization record. Ensure the image is not blurred, cropped, or covered, as this is a mandatory document.
   
5. Upload the Documents
   • Upload the completed application form (PDF) to the designated field on this page.
   • Upload the clear image of your child’s immunization record to the dedicated field.
6. Provide Your Personal Information
   Fill out your Full Name, Phone Number, and Email Address in the respective fields.
7. Add a Message (Optional)
   If you wish to provide any additional information or messages, use the “Message or Additional Information” field.
8. Submit the Form
